Sesamoiditis

Pain and Inflammation Beneath the Big Toe

Restoring Comfort and Mobility for Pain Under the Ball of the Foot

Sesamoiditis is the inflammation of the sesamoid bones—two small bones embedded within the tendons beneath the big toe joint. These bones act like pulleys, helping the big toe move smoothly when walking, running, or pushing off during activity. When irritated or overused, they can cause significant pain in the ball of the foot.

What Causes It

The condition is typically caused by repetitive stress or pressure on the forefoot. Common contributing factors include:

  • High-impact activities such as running or dancing
  • Uphill running or sudden increases in training intensity
  • High-arched feet, which place extra weight on the ball of the foot
  • Wearing high heels or shoes with thin, hard soles

Symptoms

  • Pain under the big toe joint that worsens with movement
  • Swelling or tenderness on the ball of the foot
  • Increased pain when walking barefoot or on hard surfaces

Symptoms tend to develop gradually rather than suddenly and may worsen without rest or proper footwear.

Treatment Options

The primary goal is to relieve pressure and allow the inflammation to subside. Common treatments include:

  • Rest and activity modification to avoid further irritation
  • Ice and anti-inflammatory medication to reduce pain and swelling
  • Taping or strapping the big toe to limit movement
  • Cushioned or supportive shoes to relieve pressure on the sesamoid bones

If symptoms persist, corticosteroid injections or custom orthotics may be recommended.

Prompt care can prevent chronic pain and lasting injury.
