Heel fissures, or cracked heels, occur when the skin around the heel becomes dry, thickened, and splits under pressure. Causes

The most common cause of heel fissures is xerosis (dry skin). Cracks form when pressure and friction act on hardened skin, often around existing calluses. Contributing factors include:

  • Dry skin conditions such as eczema or psoriasis
  • Diabetes or thyroid disorders that reduce skin moisture


When left untreated, the skin may split deeply, leading to pain or infection.


Symptoms

Heel fissures usually develop gradually and may appear on one or both heels. Common symptoms include:

  • Hard, thickened skin along the heel edges
  • Visible cracks or splits that may bleed
  • Pain when standing or walking
  • Redness and swelling around the heel


Severe fissures may lead to open wounds that require medical attention.


Treatment Options

Early treatment focuses on hydration, exfoliation, and protection.


Conservative Care:

  • Soak feet in warm water, then gently remove dry skin with a pumice stone
  • Apply a thick, fragrance-free moisturizer twice daily
  • Use occlusive creams or petroleum jelly at bedtime with cotton socks
  • Avoid open-backed shoes and wear footwear with proper heel support


Medical Care:
If fissures are
painful, bleeding, or infected, seek podiatric treatment. A physician may:

  • Debride thickened skin safely in-office
  • Prescribe medicated creams or dressings
  • Address underlying conditions like diabetes or fungal infections


Prevention

Keep your heels moisturized daily, avoid prolonged standing on hard surfaces, and wear well-fitting, supportive shoes. Regular podiatric care can help prevent recurrence.
