We know you have many choices when it comes to podiatric care, and we’re honored you’ve chosen The Ankle & Foot Clinic. Scheduling an appointment is simple — just call our office or request one directly through our website.


If you need to cancel or reschedule, please notify us as soon as possible so we can offer your time to another patient in need.

What to Bring

To make your visit go smoothly, please arrive 10–15 minutes early to complete your New Patient Forms in the office.


Please bring the following items with you:

  • Photo ID (driver’s license or state ID)
  • Insurance card(s)
  • List of current medications
  • Medical records or X-rays related to your foot or ankle condition
  • Payment method for any co-pays or out-of-pocket costs
