What to Bring to Your Appointment
To make your visit go smoothly, please bring the following items with you:
- Completed New Patient Forms
- Photo ID (driver’s license or state ID)
- Insurance card(s)
- List of current medications
- Medical records or X-rays related to your foot or ankle condition
- Payment method for any co-pays or out-of-pocket costs
Arriving 10–15 minutes early will help ensure we can verify your information and start your appointment on time.
